The Langmore Foundation FEES® Course

Purpose
In order to perform the FEES procedure, proper training from experienced clinicians is needed. The Langmore foundation FEES Course is an unparalleled introductory course taught by the creator of the FEES procedure and a team of expert clinicians. This course provides an opportunity to develop an understanding of the FEES procedure and to gain experience interpreting FEES for the evaluation and management of oropharyngeal dysphagia.

Course Participants
The course is primarily aimed at experienced clinicians, e.g. doctor (ENT, acute care, cancer), speech therapist, occupational therapist or nurse, who evaluate and treat patients with dysphagia using fluoroscopy, but have limited or no experience with FEES and who want to incorporate FEES into their practice. However, clinicians who are new to dysphagia and clinicians with a moderate degree of experience with FEES may also find the course useful.

Course objectives
The participant will attain a basic level of competence in performing the protocol, interpreting the results of FEES examinations and using endoscopy to guide therapy for patients with dysphagia. Each participant will pass the endoscope approximately 5 times, so he/she will become familiar with handling the endoscope passing it into the hypopharynx, and manipulating the scope within and around the larynx to attain an optimal view. Those who are already performing FEES will advance their level of competence in scoring results and applying FEES to various patient populations.

Learner Outcomes
On conclusion of the program, participants will be able to:

  1. Summarize what can be visualized and understood when using FEES.
  2. Identify normal and abnormal swallowing from endoscopic images.
  3. Describe clinical indications and risks for performing FEES.
  4. Follow a protocol for administering a FEES exam.
  5. Identify key structures, identify and interpret findings, and apply appropriate therapeutic interventions during the performance of a FEES examination.
  6. Integrate the FEES protocol into various clinical settings and with different patient populations.
  7. Describe how to use endoscopy to manage patients with dysphagia.
  8. Summarize how to implement FEES into their employment setting.
  9. Demonstrate introductory level ability passing an endoscope safely & with proper positioning to obtain an ideal view during the FEES exam.

The Langmore Advanced FEES® Course

Purpose
This is a one-day course for clinicians who already perform the Fiberoptic Endoscoopic Evaluation of Swallowing (FEES) procedure. It is meant to enhance the participants understanding of interpretation, scoring, current protocols, and the latest research in the field of dysphagia.

Course participants
The course is tailored towards clinicians who have been using FEES for at least one year, and who want more advanced training in scoring and interpreting FEES exams.

Learner Outcomes
On conclusion of the program, the participants will be able to:

  1. Summarize the latest research using endoscopy
  2. Identify normal versus abnormal swallowing
  3. Assess FEES exams using standard scoring instruments.
  4. Understand and interpret finding from FEES exams in different patient populations and in different clinical settings.
  5. Explain how and when to use endoscopy as a biofeedback tool to manage patients with dysphagia.
  6. Describe current requirements for training for FEES


Teaching Methods
Lectures and interactive case studies. The instructor will have videos to review, but participants are encouraged to bring their own videos that were challenging to interpret and/or proved challenging in recommending diet or rehabilitation options. Bring it on a USB device and present it to Dr. Langmore during first half hour of the day. Throughout the 8 hour workshop, participants will have an opportunity to sharpen their skills, share their experience and calibrate their scoring and interpretation skills against other participants and the faculty. Optimal scope handling technique, various scoring tools, and updates on FEES research will also be discussed.

Course instructor

Susan Langmore PhD, CCC-SLP, BCS-S, Professor, Boston University and Instructor of FEES courses.

Susan Langmore developed the FEES procedure in 1988. Dr. Langmore is Professor of Otolaryngology at Boston University Medical Center and Clinical Professor of Speech and Hearing Sciences at Boston University. She has carried an active clinical caseload for over 30 years, mentors BU doctoral students, conducts NIH funded clinical research, has been an editor or reviewer on more than a dozen journals, and speaks on dysphagia nationally and internationally.

Dr. Langmore has used FEES with all types of adult patients and dysphagia etiologies, and has worked in ICU, inpatient, outpatient, and nursing home settings. Dr. Langmore carries an active clinical case load, teaches graduate and doctoral students, conducts extensive research, and lectures on dysphagia nationally and internationally. She is the author of the textbook Endoscopic Evaluation and Treatment of Swallowing Disorders.
